Community Care Management

A therapist will come to the home or youth’s school 1-3 times weekly (depending on needs) to support and engage the child and family in counseling. They will work to identify the root causes that created the crisis. The family will look at communication patterns, expectations, triggers of crisis, and coping skills they can use in the future. The therapist will ensure the youth’s mental health and emotional needs are evaluated regularly during this time of Community Care Management. The therapist will link the family with necessary support services (including urgent psychiatry medication assessments/reviews here at Wedgwood Christian Services, if necessary) and encourage the youth and family to continue counseling through traditional office-based services if warranted once Community Care Management is completed.

Priority Health Commercial insurance members with a child under 18 that has symptoms that put him/her at risk for hospitalization

Primarily the clinician visits the client/family either at home or the school, but meeting at the Wedgwood Bouma Counseling Center or through Telehealth are also options depending on circumstance and need.
