Wedgwood’s In-home Services exist to promote physical, social, intellectual, emotional and spiritual growth and health for each child, adult and family within the community. Ourcommunity-based services are available to respond to a wide range of needs present in our communities. The aim of all services is to support and enhance the meaningful participation of clients and families within their communities. Treatment can be customized to meet the unique needs of individual children and families. In-Home Services provide support and therapeutic intervention to prevent out-of-home placement, or assist in reintegration following out-of-home placement. In-Home Services may include individual and family counseling, mentoring, and phone contacts with the family, and on their behalf, connecting them with community resources that can provide additional assistance. The treatment plan and number/type of contacts are developed on the basis of each family’s needs, and can involve the use of Youth Treatment Specialists as well as therapists. Family Case Management Program: This is a service for families with children under the age of 18 who have a severe emotional disturbance. Services are provided in the home or other community location agreed upon with the family a minimum of twice monthly. Services are family-centered; identify and build on the strengths of the family; provide support and encouragement; coordinate care with schools and primary care doctors; coordinate psychiatric treatment when needed; assist parents with accessing crisis services; and assist the family to expand their support system. Ages: 3 - 18 years Home-Based Services Program: This is an intensive service for families with children under the age of 18 who have a severe emotional disturbance. Services are family-specific and are individualized by the culture, strengths, concerns and resources of each family. Services provide an evaluation and medication reviews by an agency psychiatrist, a minimum of 2 hours in-person contact per week, individual, family and group treatment, crisis services, mentor and paraprofessional services, and respite services (this is a type of support available that is provided on an intermittent or short-term basis to provide the parent with a break from caring for their child with emotional or behavioral challenges). Ages: 3 - 18 years Juvenile Community Justice Program (JCJ): The Juvenile Community Justice Program is a home-based balanced restorative justice (BARJ) treatment program that works with adolescent offenders and their families in their own homes after they have been discharged from an intensive out of home/ residential setting. The program supports the reintegration of the youth into the community by providing intensive home-based treatment and surveillance services. Ages: 3 - 18 years Substance Abuse Disorder (SUD) Outreach: This is a home-based program designed to provide community based short-term individual and family treatment for youth 12-18 years old. The program consists of 12-14 sessions (10 individual sessions with the adolescent and 4 sessions with the parent/guardian). Goals of this approach include abstinence, pro-social activities, relapse prevention, positive relationships with peers and family, problem solving and anger management skills. This is a more intensive program for youth identified with an existing substance abuse problem. Ages: 12 - 18 years Adolescent Community Reinforcement (ACRA): This is a home-based program designed to provide community based services for youth 12-15 year olds with substance use and truancy/expulsion issues. ACRA is a systems approach with the therapist partnering with the family, school, and family support system in the delivery of services. The Kent Intermediate School District Office of Truancy and the Kent County Circuit Court-Family Division Crisis Intervention Program partner with Network180 and Wedgwood Christian Services to identify appropriate adolescents for this service.
